Eligibility Requirements

Age Requirement
Must be 18 years or older and on Medical Assistance (MA).

Documented Disability
Have a documented disability such as aging, blindness, mental illness, or developmental disability.

Assistance Needs
Require help with communication, mobility, decision-making, or managing challenging behaviors.

Housing Instability
Experiencing homelessness, transitioning, or at risk of losing housing.

Risk of Institutionalization
At risk of institutionalization and meeting care level criteria, eligible for specific waivers (BI, CADI, CAC, DD, EW).
